Job summary

Barre Gardens Nursing and Rehab in Barre, VT seeks an Occupational Therapist (OT) – PRN to provide quality rehabilitation services within our skilled nursing facility. You will evaluate residents, develop individualized treatment plans, and deliver therapy to maximize independence and quality of life.

Responsibilities include data collection for treatment planning, implementing OT programs, coordinating with the rehab team, educating residents and families, monitoring progress, and maintaining

Qualifications

• Degree in Occupational Therapy from an accredited program.
• Active Vermont OT license (OTR/L).
• Strong communication and interpersonal skills.
• Skilled nursing, rehabilitation, or long-term care experience preferred, but not required.

Responsibilities

• Evaluate residents to obtain data for treatment planning and set measurable goals.
• Implement occupational therapy treatment programs based on residents' needs.
• Collaborate with the rehabilitation team to coordinate therapy programs.
• Instruct residents, families, and caregivers in OT techniques and plans.
• Monitor progress and adjust treatment plans as needed.
• Maintain accurate, timely documentation per facility and regulatory requirements.
• Participate in interdisciplinary care planning and communication.
