Job summary

Nexus Health is seeking an Occupational Therapist in Illinois to develop individualized treatment plans for patients in SNF/long-term care settings. The role emphasizes maximizing safety and daily living activities, with inter-disciplinary team collaboration and strict documentation standards.

Qualified candidates hold an IL OT license, with preferred SNF experience and familiarity with Net Health EDS. This on-site position offers growth within a team-oriented environment and a strong benefits

Qualifications

• Active/Valid Illinois license in Occupational Therapy.
• SNF/Long term care experience preferred.
• Current knowledge of treatment practices.
• Knowledge of Net Health Electronic Documentation System preferred.

Responsibilities

• Develop personalized treatment plans to maximize safety and performance in ADL and IADL.
• Ensure all treatment delivered follows the established plan of care.
• Monitor patient response and modify treatment with IDT collaboration.
• Document thoroughly and timely in compliance with company and state/federal regulations.

Occupational Therapist Responsibilities:

• Developing personalized treatment plans using a variety of treatment techniques to maximize safety and performance in activities of daily living (ADL) and instrumental activities of daily living (IADL).
• Ensure all treatment delivered is in accordance with the established plan of care.
• Monitor patient response to treatment intervention and modify treatment approach and directive as clinically appropriate with IDT collaboration.
• Ensure documentation is completed thoroughly and timely and complies with all company and state/federal regulatory requirements.

Requirements

Occupational Therapist Qualifications:

• Active/Valid Illinois license in Occupational Therapy
