Job summary

Creative Solutions In HealthCare is seeking a compassionate PRN Occupational Therapist to assess and treat residents in a skilled nursing setting, focusing on independence and quality of life. You will devise individualized plans to improve daily functioning and collaborate with an interdisciplinary team to ensure optimal outcomes.

Licensed OT in Texas with SNF or long-term care exposure is preferred. Strong communication, flexibility in schedule, and the ability to work autonomously are

Qualifications

• Active Texas OT license in good standing.
• SNF or long-term care experience preferred.
• Knowledge of OT treatment practices.
• Experience with Net Health EDS.
• Strong communication and collaboration skills.

Responsibilities

• Consult with patients to evaluate their physical condition and its impact on daily occupations.
• Develop individualized treatment plans to maximize ADLs and IADLs.
• Assess home environments and recommend modifications based on needs and capabilities.
• Recommend adaptive equipment and provide training to support independence and safety.
• Collaborate with interdisciplinary team to ensure high-quality patient care.
• Complete documentation thoroughly and timely in compliance with policies and regulations.

Join Our Team as a (PRN) Occupational Therapist (OT)

We are seeking a compassionate and flexible Occupational Therapist (OT) to join our in-house team of outstanding professionals on a PRN basis. In this role, you will provide person-centered care focused on improving residents’ independence, safety, and quality of life through meaningful daily activities and functional outcomes.

Your Impact as an Occupational Therapist (OT)

• Consult with patients to evaluate their physical condition and its impact on daily occupations, well-being, and quality of life
• Assess and interpret patient evaluations and test results
• Develop individualized treatment plans using a variety of therapeutic techniques to maximize performance in activities of daily living (ADLs) and instrumental activities of daily living (IADLs)
• Assess patients’ home and living environments and recommend modifications based on their needs and capabilities
• Recommend adaptive equipment and provide training to support independence and safety
• Ensure all treatments are delivered in accordance with the established plan of care
• Monitor patient response to treatment and modify interventions as clinically appropriate in collaboration with the interdisciplinary team
• Provide clinical education and support to patients, family members, and caregivers
• Collaborate with interdisciplinary team members to ensure high-quality patient care
• Complete documentation thoroughly and timely in compliance with company policies and state/federal regulations
• Comply with all applicable rules, regulations, and procedures
• Attend required meetings as designated by the Director of Rehab

What Makes You a Great Fit

• Active and valid Texas Occupational Therapy license
• In good standing with all regulatory agencies and licensing boards
• Skilled Nursing Facility (SNF) or long-term care experience preferred
• Current knowledge of occupational therapy treatment practices
• Experience with Net Health Electronic Documentation System preferred
• Ability to manage patients with varying personalities and needs
• Strong communication, collaboration, and clinical decision-making skills
