Job summary

Bria Health Services, L.L.C. is seeking a PRN Occupational Therapist to support skilled nursing facility clients in Illinois. You will develop personalized treatment plans, monitor progress, and collaborate with an interdisciplinary team to maximize safety and independence in ADLs/IADLs.

Responsibilities include documenting in accordance with plan of care and regulatory requirements. Illinois OT license and SNF/LTC experience preferred; Net Health EDS knowledge a plus.

Qualifications

• Active Illinois OT license required.
• SNF/LTC experience preferred.
• Knowledge of state/federal regulatory requirements for documentation.

Responsibilities

• Develop personalized treatment plans to maximize safety and independence in ADLs/IADLs.
• Ensure all treatment delivered follows the established plan of care.
• Monitor patient response and modify treatment with IDT collaboration.
• Ensure documentation is thorough and timely and complies with regulatory requirements.

PRN Occupational Therapist Responsibilities:

• Developing personalized treatment plans using a variety of treatment techniques to maximize safety and performance in activities of daily living (ADL) and instrumental activities of daily living (IADL).
• Ensure all treatment delivered is in accordance with the established plan of care.
• Monitor patient response to treatment intervention and modify treatment approach and directive as clinically appropriate with IDT collaboration.
• Ensure documentation is completed thoroughly and timely and complies with all company and state/federal regulatory requirements.

Requirements

PRN Occupational Therapist Qualifications:

• Active/Valid Illinois license in Occupational Therapy
• SNF/Long term care experience preferred.
• Current knowledge of treatment practices
• Knowledge of Net Health Electronic Documentation System preferred.
