Job summary

South Creek Post Acute in Washington state is seeking a licensed Occupational Therapist for PRN coverage to evaluate and treat residents, develop treatment plans, and coordinate with families and physicians.

The OT will supervise COTA, maintain documentation in Net Health and PointClickCare, educate residents and caregivers, and participate in care conferences while upholding infection control and safety standards.

Qualifications

• Licensed OT required with current state licensure.
• Bachelor’s degree in OT; masters/doctorate preferred.
• Experience supervising aides/assistants is a plus.

Responsibilities

• Evaluate residents to determine OT needs and rehab potential.
• Develop and update individualized treatment plans to improve mobility and independence.
• Supervise COTA and aides in resident care and activities.
• Maintain thorough documentation per facility and payer requirements.
• Educate residents, families, and staff on goals and home exercise plans.
