Job summary

Agape Hospice & Homecare in Arizona is hiring a licensed Occupational Therapist to join our home health team serving Pinal County. This full-time role focuses on individualized therapy plans, patient independence, and collaborative care with a compassionate team.

You will assess patients, teach adaptive skills, document progress, and coordinate with physicians to ensure quality outcomes. Requirements include AZ OT license, ACOTE-accredited program completion, NBCOT eligibility, CPR, and reliable

Qualifications

• Actively licensed as an Occupational Therapist (OT) in the State of Arizona.
• Graduate of an ACOTE-accredited OT program, eligible for NBCOT certification.
• Minimum 2 years’ clinical experience (home health preferred).
• Current CPR certification.
• Valid driver’s license, insurance, and reliable transportation.

Responsibilities

• Develop and implement individualized care plans.
• Provide therapy services per physician’s orders.
• Assess functional status and recommend adaptive equipment.
• Teach new skills or retrain patients in daily activities.
• Maintain accurate documentation and participate in case conferences.
