Job summary

Heritage of Clara Barton Assisted Living Community in New Jersey is seeking a passionate PRN Occupational Therapist to join our rehabilitation team. You will evaluate and treat seniors in an assisted living setting, developing personalized plans to improve strength, balance and functional mobility.

You will collaborate with physicians, nurses and other therapists, document progress in the EMR, participate in care conferences, and supervise COTAs as needed.

Qualifications

• Active Occupational Therapist license in the state of New Jersey (or eligible to obtain)
• New grads welcome! We provide excellent mentorship and training
• Previous experience in SNF, long-term care, assisted living or geriatrics is a plus
• Strong communication and interpersonal skills
• Ability to work collaboratively as part of an interdisciplinary team
• Compassionate, positive attitude with a genuine desire to make a difference in residents’ lives

Responsibilities

• Develop personalized treatment plans to improve strength, balance, ADL performance, and functional mobility
• Collaborate closely with physicians, nurses, physical therapists, and speech therapists
• Document patient progress accurately and in a timely manner using our electronic medical records system
• Participate in care plan meetings and family conferences as needed
• Provide compassionate, patient-centered care in a supportive team environment
• Supervise COTAs as needed
• Report directly to the Director of Rehabilitation
