Job summary

Creative Solutions In HealthCare is seeking a compassionate Occupational Therapist (OT) to join our in-house rehab team on a PRN basis. You will evaluate residents, plan individualized therapies, and work with an interdisciplinary team to improve ADLs and safety.

You will implement evidence-based OT treatments, monitor outcomes, and educate patients and families while documenting thoroughly per policy. Prior experience in SNF settings is preferred and you must hold an active Texas OT license.

Qualifications

• Active Texas OT license in good standing.
• SNF or long-term care experience preferred.
• Knowledge of OT treatment practices and documentation.
• Experience with Net Health Electronic Documentation System preferred.

Responsibilities

• Consult with patients to evaluate their condition and how it affects daily activities.
• Assess evaluations and test results and interpret findings.
• Develop individualized treatment plans to maximize ADLs/IADLs.
• Assess home environments and recommend modifications for safety.
• Recommend adaptive equipment and provide training to support independence.
• Deliver treatments according to the plan of care and adjust as needed.
• Monitor patient response and modify interventions with the interdisciplinary team.
• Provide education and support to patients, families, and caregivers.
• Collaborate with team to ensure high-quality care.
• Complete documentation thoroughly and in line with policies.
• Comply with all rules and attend required meetings.

Join Our Team as a (PRN) Occupational Therapist (OT)

We are seeking a compassionate and flexible Occupational Therapist (OT) to join our in-house team of outstanding professionals on a PRN basis. In this role, you will provide person-centered care focused on improving residents’ independence, safety, and quality of life through meaningful daily activities and functional outcomes.

Your Impact as an Occupational Therapist (OT)

• Consult with patients to evaluate their physical condition and its impact on daily occupations, well-being, and quality of life
• Assess and interpret patient evaluations and test results
• Develop individualized treatment plans using a variety of therapeutic techniques to maximize performance in activities of daily living (ADLs) and instrumental activities of daily living (IADLs)
• Assess patients’ home and living environments and recommend modifications based on their needs and capabilities
• Recommend adaptive equipment and provide training to support independence and safety
• Ensure all treatments are delivered in accordance with the established plan of care
• Monitor patient response to treatment and modify interventions as clinically appropriate in collaboration with the interdisciplinary team
• Provide clinical education and support to patients, family members, and caregivers
• Collaborate with interdisciplinary team members to ensure high-quality patient care
• Complete documentation thoroughly and timely in compliance with company policies and state/federal regulations
• Comply with all applicable rules, regulations, and procedures
• Attend required meetings as designated by the Director of Rehab

What Makes You a Great Fit

• Active and valid Texas Occupational Therapy license
• In good standing with all regulatory agencies and licensing boards
• Skilled Nursing Facility (SNF) or long-term care experience preferred
• Current knowledge of occupational therapy treatment practices
• Experience with Net Health Electronic Documentation System preferred
• Ability to manage patients with varying personalities and needs
• Strong communication, collaboration, and clinical decision-making skills
