Job summary

Nexus at Wood River is seeking an Occupational Therapist to evaluate and treat residents, helping them regain independence in daily living activities. You will develop personalized plans and collaborate with the IDT to adjust care as needed.

Illinois licensure is required, with preference for SNF/long-term care experience and knowledge of PCC documentation. This on-site role offers a supportive, team-focused environment with ongoing professional development opportunities.

Qualifications

• Active IL license in Occupational Therapy.
• SNF/Long term care experience preferred.
• Knowledge of current treatment practices.
• Familiarity with PCC Electronic Documentation is preferred.

Responsibilities

Position Overview:

Evaluate and treat residents to help them regain, maintain, or improve the skills needed for daily living and functional independence.

Key Responsibilities:

• Developing personalized treatment plans using a variety of treatment techniques to maximize safety and performance in activities of daily living (ADL) and instrumental activities of daily living (IADL).
• Ensure all treatment delivered is in accordance with the established plan of care.
• Monitor patient response to treatment intervention and modify treatment approach and directive as clinically appropriate with IDT collaboration.
• Ensure documentation is completed thoroughly and timely and complies with all company and state/federal regulatory requirements.
