Nexus at Geneva is seeking an Occupational Therapist to evaluate residents and deliver treatments that help them regain or improve independence in daily living. The role requires developing individualized plans, monitoring progress, and ensuring documentation complies with care plans and regulations.
Preferred candidates hold an Illinois OT license and have SNF/long-term care experience, with knowledge of PCC documentation systems.
Position Overview:
Evaluate and treat residents to help them regain, maintain, or improve the skills needed for daily living and functional independence.
Key Responsibilities:
Developing personalized treatment plans using a variety of treatment techniques to maximize safety and performance in activities of daily living (ADL) and instrumental activities of daily living (IADL).
Ensure all treatment delivered is in accordance with the established plan of care.
Monitor patient response to treatment intervention and modify treatment approach and directive as clinically appropriate with IDT collaboration.
Ensure documentation is completed thoroughly and timely and complies with all company and state/federal regulatory requirements.

Preferred Qualifications:
Active/Valid Illinois license in Occupational Therapy
SNF/Long term care experience preferred.
Current knowledge of treatment practices
Knowledge of PCC Electronic Documentation System preferred.
