Job summary

Nexus at Wood River in Illinois is seeking an Occupational Therapist to evaluate and treat residents, helping them regain, maintain, or improve skills for daily living and functional independence. The role focuses on developing patient-centered treatment plans and ensuring care aligns with established goals.

You will collaborate with the interdisciplinary team, monitor progress, adjust strategies as needed, and document thoroughly to meet regulatory standards.

Qualifications

• Active/Valid Illinois license in Occupational Therapy.
• SNF/Long term care experience preferred.
• Current knowledge of treatment practices.
• Knowledge of PCC Electronic Documentation System preferred.

• Develop personalized treatment plans using a variety of treatment techniques to maximize safety and performance in activities of daily living (ADL) and instrumental activities of daily living (IADL).
• Ensure all treatment delivered is in accordance with the established plan of care.
• Monitor patient response to treatment intervention and modify treatment approach and directive as clinically appropriate with IDT collaboration.
