Job summary

Chatham Hills Subacute Care Center is seeking a dedicated Occupational Therapist to join our rehabilitation team in the New Jersey area. The OT will assess patients, establish treatment programs, and work under physician direction to meet patient goals.

The role includes supervising Occupational Therapy Assistants, documenting progress, and collaborating with the care team to deliver patient-centered treatment in a long-term care setting. An active OT license is required.

Qualifications

• Requires graduation from an accredited Occupational Therapist program.
• Must hold a valid OT license and registration for state of practice.
• Experience supervising Occupational Therapy Assistants a plus.

Responsibilities

• Assess referred patients needing rehab and provide direct care.
• Develop and administer individualized treatment plans with goals.
• Document patient progress and educate patients and families.
