Job summary

Chatham Hills Subacute Care Center is seeking an Occupational Therapist to join our growing team. The OT will assess patients in need of rehabilitation, develop and implement treatment programs, and educate patients and families on therapy approaches.

The role requires graduation from an accredited OT program, active state licensure, and experience supervising assistants. Long-term/geriatric care experience is preferred; strong documentation and teamwork are essential.

Qualifications

• Must have graduated from an accredited Occupational Therapist program.
• Must have a valid license and registration for state of practice.
• Experience supervising Occupational Therapy Assistants; long-term/geriatric care preferred.

Responsibilities

• Assess referred patients to determine rehabilitation needs and goals.
• Administer and document treatment plans under physician direction.
• Educate patients and families on therapy methods.
• Collaborate with all disciplines to provide patient-centered care.
• Ensure HIPAA confidentiality and professional conduct.
