Job summary

Chatham Hills Subacute Care Center seeks a dedicated Occupational Therapist to join our rehabilitation team in Maplewood, NJ. The OT evaluates referred patients, designs individualized treatment plans, and delivers direct patient care to improve function and independence.

Responsibilities include progression of therapy, documentation, and family education, with collaboration across disciplines to ensure superior patient outcomes in a long-term care setting.

Qualifications

• Graduated from an accredited OT program.
• Valid state OT license required.
• Experience supervising OTAs is expected.
• Long-term/geriatric care experience preferred.
• Strong documentation skills are essential.
• Demonstrates professional conduct at all times.

Responsibilities

• Assess referred patients needing rehabilitation.
• Develop and implement treatment plans with patient goals.
• Document patient progress and outcomes.
• Educate patients and families on therapy approaches.
• Coordinate with the rehabilitation team to optimize care.
