Job summary

Bear Mountain Healthcare seeks an Occupational Therapist Registered (OTR) for the West Roxbury Health and Rehabilitation Center. The OTR will join an in-house rehab team to evaluate residents, devise customized treatment plans, and strive for maximum independence and quality of life in a skilled nursing setting.

Responsibilities include plan/implement OT programs, document thoroughly, communicate results to the care team, and initiate appropriate referrals.

Qualifications

• Must have an active state license or be license eligible in Occupational Therapy.
• Education requirement: BS, MS, or PhD in Occupational Therapy from an accredited program.

Responsibilities

• Plan, develop, organize, implement, evaluate and direct Occupational Therapy Services in accordance with current regulations.
• Maintain accurate documentation and department records.
• Ensure effective communication of OT programs.
• Initiate referrals per OT procedures.
• Conduct resident screenings to determine need for intervention.
• Communicate evaluation findings to residents, families and team members.
