In this role, you’ll help residents improve **function, independence, and quality of life** while collaborating with an interdisciplinary care team.### **Key Responsibilities*** Evaluate residents and develop individualized **treatment plans*** Provide therapy services per **physician orders and care goals*** Document evaluations, treatments, and progress notes accurately and timely* Collaborate with physicians, nursing, and therapy team members* Educate residents, families, and caregivers on therapy programs* Participate in **care planning and discharge planning*** Conduct **in-services and staff training** as needed* Ensure compliance with all **state, federal, and payer guidelines**### **Qualifications*** Degree in Occupational Therapy (Bachelor’s required; Master’s/Doctorate preferred)* Active **South Carolina OT license (OTR)** in good standing* Experience in **skilled nursing or rehab setting preferred*** Familiarity with **PointClickCare (PCC) and Casamba** preferred* Strong documentation, communication, and clinical skillsWe are an equal opportunity employer.
