Job summary

At Canyon Creek Post-Acute in Castro Valley, CA, the Occupational Therapist provides and directs all aspects of patient care, including evaluation, treatment planning, goal setting, family education and documentation in line with facility guidelines and professional standards.

Collaborates with a multidisciplinary team to develop individualized care plans, ensures accurate coding and documentation, and educates staff on rehabilitation benefits to maximize patient outcomes while maintaining

Qualifications

• Active license as Occupational Therapist in California.
• BLS/CPR certification.
• Experience in long-term care preferred.
• Graduate of an approved OT curriculum.

Responsibilities

• Perform evaluations per MD orders and payer guidelines.
• Develop treatment plans and provide therapy to optimize function.
• Document evaluations and progress notes.
• Educate staff and payers on benefits of intervention.
• Maintain licensure and credentialing.
