Job summary

White Blossom Care Center in San Jose, CA is seeking an Assistant Director of Rehab (ADOR) and Occupational Therapist (OTR) to lead a high-volume therapy team within an in-house rehab program. You will split time between direct patient care and supporting the DOR in daily gym operations and staff mentoring.

The ideal candidate is an experienced OTR with leadership experience, proficient in Medicare PDPM, and able to ensure clinical excellence and compliant documentation.

Qualifications

• Must be a licensed Occupational Therapist in California (OTR).
• Minimum 2–3 years of clinical SNF or post-acute care experience; leadership preferred.
• Strong understanding of Medicare PDPM guidelines and documentation systems.
• Ability to supervise, mentor, and coordinate a large therapy team.

Responsibilities

• Provide clinical leadership in collaboration with DOR.
• Assist with gym operations, scheduling, and compliance monitoring.
• Conduct comprehensive OT evaluations and deliver evidence-based interventions.
• Oversee diverse caseload including orthopedic, neuro, cardiac, and vascular rehab.
• Ensure timely and accurate documentation and recertifications.
• Coordinate DME ordering and family training for safe discharges.
