Job summary

White Blossom Care Center in San Jose, CA is seeking a dedicated Occupational Therapist (OTR) to join our in-house rehabilitation team. Work in a high-volume, resource-rich environment with a spacious gym and ample growth opportunities.

The role offers a flexible schedule (FT/PT/PRN) with on-site modalities like E-Stim, Ultrasound, and Diathermy, plus mentorship and ongoing clinical education. New graduates are encouraged to apply.

Qualifications

• Current and valid California OT license in good standing.
• Experience in post-acute, skilled nursing, or long-term care preferred.

Responsibilities

• Conduct comprehensive occupational therapy evaluations and develop individualized treatment plans based on physician orders and patient needs.
• Provide evidence-based occupational therapy interventions focused on improving functional mobility, independence, safety, and quality of life.
• Supervise and support Certified Occupational Therapy Assistants (COTAs) in accordance with California regulations and applicable standards of practice.
• Participate in interdisciplinary care conferences, utilization reviews, and rehabilitation meetings.
• Complete timely and accurate evaluations, daily documentation, progress notes, and recertifications.
• Assess patients for appropriate durable medical equipment (DME) and provide training to promote safety and independence.
• Educate patients, families, caregivers, and nursing staff on therapeutic strategies and carryover programs.
• Participate in discharge planning and caregiver training to support successful transitions of care.
