Resolve Solutions-Harmony is seeking a compassionate Occupational Therapist (OTR) to join our rehabilitation team in Phoenix, Arizona. The role involves evaluating residents, developing individualized treatment plans, and helping residents regain independence in daily activities within a skilled nursing facility setting.

Full-time OTRs are eligible for a $10,000 sign-on bonus, paid in increments. CPR certification and an Arizona license are required; SNF experience is preferred.

Qualifications

• Current Arizona Occupational Therapist license is required.
• Graduate from an accredited Occupational Therapy program.
• Current CPR certification is required.
• Experience in skilled nursing or long-term care is preferred.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide occupational therapy services based on residents' needs and goals.
• Help residents improve daily living activities.
• Document evaluations, treatments, progress, and outcomes.
• Collaborate with physicians, nursing staff, therapists, residents, and families.
• Educate residents, caregivers, and team members on treatment recommendations.
• Maintain compliance with facility, state, and federal requirements.

Qualifications
