Resolve-Harmony-Center in Phoenix, AZ is seeking a compassionate Occupational Therapist (OTR) to join our rehabilitation team in a skilled nursing facility. You will evaluate residents, develop treatment plans, and help them regain independence with targeted daily activities and goals.

Qualifications

• Current Arizona Occupational Therapist license
• Graduate of an accredited Occupational Therapy program
• Current CPR certification
• Skilled nursing or long-term care experience preferred
• Strong communication, documentation, and clinical decision-making skills

Responsibilities

• Evaluate residents and develop individualized treatment plans
• Provide occupational therapy services based on each resident’s needs and goals
• Help residents improve their ability to perform daily activities
• Document evaluations, treatments, progress, and outcomes accurately
• Collaborate with physicians, nursing staff, therapists, residents, and families
• Educate residents, caregivers, and team members on treatment recommendations
• Maintain compliance with facility, state, and federal requirements

We are seeking a compassionate and motivated Occupational Therapist (OTR) to join our rehabilitation team in Phoenix, Arizona. This is an excellent opportunity to help residents regain independence, improve daily living skills, and achieve meaningful functional goals.

Full-time OTRs are eligible for a $10,000 sign-on bonus, paid in increments. Additional eligibility requirements and payout terms apply.
