Valley Manor Health & Rehabilitation Center is seeking a licensed Occupational Therapist (OTR/L) to join our rehabilitation team. The role emphasizes resident-centered care, evaluation of physical and cognitive abilities, and development of individualized OT plans.

Join a compassionate interdisciplinary team within a long-standing post-acute care facility. Active PA OT license and NBCOT certification are required, with preference for skilled nursing or rehab experience.

Qualifications

• Current and active Pennsylvania OT license.
• Graduate of an accredited OT program.
• NBCOT certification.
• Experience in skilled nursing or rehabilitation preferred.
• Strong communication and teamwork abilities.

Responsibilities

• Evaluate residents’ physical, cognitive, and functional abilities.
• Develop and implement individualized OT treatment plans.
• Provide therapeutic interventions to improve ADLs, mobility, coordination, strength, and independence.
• Educate residents, families, and caregivers on treatment plans, adaptive techniques, and equipment use.
• Monitor resident progress and adjust treatment plans as needed; complete documentation.
• Participate in care planning meetings and interdisciplinary team conferences.
• Recommend adaptive equipment and environmental modifications.
• Collaborate with physicians, nursing staff, therapists, and care team members.
• Ensure services comply with facility policies and state/federal regulations.

Responsibilities

• Evaluate residents’ physical, cognitive, and functional abilities

• Provide therapeutic interventions to improve activities of daily living, mobility, coordination, strength, and independence
• Educate residents, families, and caregivers on treatment plans, adaptive techniques, and equipment use

• Participate in care planning meetings and interdisciplinary team conferences
