Job summary

HarmonyPark Wilson is seeking a licensed Occupational Therapist to provide skilled therapy services to residents in accordance with physician orders and care plans. This role focuses on improving resident function, mobility, communication, and quality of life.

This position offers a $10,000 sign-on bonus for full-time eligibility and flexibility with full-time, part-time, or PRN schedules. The ideal candidate is patient-focused, collaborates with nursing and interdisciplinary teams, and strives

Qualifications

• Must have OT degree from an accredited program.
• Current OT licensure in the state.
• Experience in long-term care preferred.
• Strong documentation and communication skills.
• Patient-focused and team-oriented.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide therapy services following physician orders.
• Document evaluations, treatments, progress notes, and discharge summaries.
• Collaborate with nursing and interdisciplinary teams.
• Participate in care planning and utilization review meetings.
• Educate residents, families, and staff on therapy goals and plans.
• Maintain productivity and documentation standards.
• Support regulatory compliance and survey readiness.
