Job summary

Rehab Strategies in Indiana is seeking a compassionate Occupational Therapist to assess, plan, and implement therapy interventions that promote independence in self-care, mobility, and meaningful activities. You will conduct patient evaluations, establish treatment goals, and collaborate with nursing, PT, SLP, and families to maximize outcomes while ensuring timely documentation and compliance.

Qualifications

• Current state license as an Occupational Therapist.
• Graduate of an accredited OT program with Bachelor’s, Master’s, or Doctorate Degree.
• Strong organizational and problem-solving skills.
• Experience in Skilled Nursing/Long-Term Care preferred.

Responsibilities

• Conduct patient evaluations and establish treatment goals.
• Implement therapy interventions to improve ADLs, fine motor skills, and functional mobility.
• Recommend adaptive equipment and environmental modifications as needed.
• Collaborate with nursing, PT, SLP, and families to maximize patient outcomes.
• Educate patients and caregivers on strategies for success at home and in the community.
• Complete required documentation in a timely, accurate manner.
