Vasona Creek Healthcare in Los Gatos, CA is seeking a dedicated Occupational Therapist (OTR) to join our short-term rehabilitation team. You will develop individualized treatment plans, supervise OT assistants, and collaborate with a multidisciplinary team to help patients regain function.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide OT services and supervise OT assistants per California Act.
• Document evaluations, progress notes, treatment sessions.
• Collaborate with nursing, therapy, physicians, supervisors, and other team members.
• Participate in discharge planning and rehabilitation conferences.
• Educate staff and families on restoration programs and techniques.
• Assist with durable medical equipment recommendations for safety.
• Support facility in-service training and safety reporting.
