In this role, you will evaluate residents, develop individualized treatment plans, and provide therapy services that help residents achieve their highest level of independence and quality of life.## Key Responsibilities* Evaluate residents and develop individualized treatment plans based on physician orders and clinical needs* Provide skilled occupational therapy services in accordance with established goals* Monitor resident progress and modify treatment plans as appropriate* Communicate effectively with physicians, nursing staff, residents, families, and interdisciplinary team members* Document evaluations, treatment sessions, progress notes, and discharge summaries accurately and timely* Participate in care planning meetings, rehabilitation conferences, and discharge planning* Educate residents, families, and caregivers on treatment goals and home programs* Conduct in-service training and educational programs for facility staff* Ensure compliance with all state practice acts, regulatory requirements, and facility policies* Promote resident independence, safety, and overall well-being## ## Qualifications### Education & Experience* Bachelor's degree in Occupational Therapy required* Master's or Doctorate degree in Occupational Therapy preferred* Experience in skilled nursing, rehabilitation, or long-term care preferred* Experience with **PointClickCare (PCC)** and **Casamba** preferred* Strong communication, documentation, and organizational skills### Licenses & Certifications* Current **South Carolina Occupational Therapist license** in good standing* Must maintain all required continuing education requirements* Must remain in good standing with the State Board## ## What Makes You a Great Fit* Compassion for an underserved population* Strong clinical judgment and problem-solving skills* Excellent communication and interpersonal abilities* Ability to work independently and collaboratively within an interdisciplinary team* Commitment to resident-centered care and positive outcomes* Passion for helping residents achieve their highest level of independence## ## Apply TodayJoin a therapy team dedicated to helping residents regain function, improve independence, and enhance their quality of life.
