Job summary

Harvest Crossing Post Acute in Manteca, CA seeks an Occupational Therapist to evaluate residents, design therapy plans, and improve daily living activities, mobility, and safety. You will collaborate with a multidisciplinary team in a skilled nursing facility and supervise assistants as needed.

Qualifications

• Currently completing or recently graduated from an approved Occupational Therapy program.
• Must be eligible to obtain California Occupational Therapist license prior to providing services.
• Strong communication, documentation, and team collaboration skills.

Responsibilities

• Evaluate patients per physician orders and facility guidelines.
• Develop and implement OT plans of care to improve ADLs, mobility, and safety.
• Supervise assistants and coordinate with the rehab team.
