Job summary

Harvest Crossing Post Acute is hiring an Occupational Therapist (OT) to evaluate and treat residents in our skilled nursing facility. You will develop plans of care and provide therapy focused on independence in daily living, mobility, strength, balance, and cognition.

We welcome new grads and experienced therapists; California licensure eligibility and CPR are required. Excellent team collaboration, documentation, and patient education are essential in this role.

Qualifications

• Must have or be eligible to obtain California OT license.
• CPR certification required.
• SNF experience beneficial but not required.
• Strong documentation and organizational skills.
• Proficiency with EMR and rehabilitation systems; willingness to learn.

Responsibilities

• Evaluate patients according to physician orders and facility guidelines.
• Develop individualized OT plans of care based on evaluation findings and goals.
• Provide OT interventions to improve ADLs, mobility, strength, balance, cognition, and safety.
• Supervise Certified Occupational Therapy Assistants and rehabilitation staff.
• Coordinate treatment with the interdisciplinary team and communicate patient progress.
• Enter patient data into EMR and ensure timely documentation.
• Assist with transitions, education for patients/families, and discharge planning.
