Job summary

Centra Healthcare Solutions is seeking an Occupational Therapist (OT) for a Skilled Nursing Facility / Long-Term Care setting in Modesto, CA. This is a part-time, direct hire position offering the opportunity to contribute to patient-centered OT care within a supportive, mission-driven team.

The OT will assess patient needs, develop treatment plans, implement therapies, and monitor progress while coordinating with a multidisciplinary team to achieve optimal outcomes in line with physician orders.

Qualifications

• Must have graduated from an accredited school.
• Current state license as an OT or actively pursuing licensure.
• Current CPR certification.
• Provide skilled OT services/interventions per physician orders.
• Assess patient needs, plan, evaluate and modify care to meet OT goals.
• Collaborate with all disciplines to plan and evaluate team goals.

Job Description:

A great Skilled Nursing Facility (SNF) is looking for an experienced or newly graduated Occupational Therapist / OT to join their growing team for a direct hire / 'perm' assignment!

• This is a direct hire / 'perm' position.
• Days schedule.
• This role assumes responsibility and accountability for incorporating the vision, values, mission and critical goals of the organization into their job performance.

Requirements include:

• Must have graduated from an accredited school.
• Current state license as an Occupational Therapist / OT or proactively in the process of the application process for current state licensure as an Occupational Therapist / OT.
• Current CPR certification.
• Provide skilled occupational therapy services / interventions in accordance with physician orders.
• Assesses patient needs, plans for, evaluates and modifies care to meet goals of occupational therapy interventions.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.
