Job summary

Centra Healthcare Solutions is seeking an Occupational Therapist (OT) for a contract position in San Rafael, CA. The role involves direct patient care in a SNF/LTC setting, evaluating patients, and implementing therapy plans under physician orders.

Qualified candidates will have graduated from an accredited OT program and hold or be pursuing state licensure. Collaboration with the care team and education for patients and families are key aspects of the job.

Qualifications

• Graduate from an accredited occupational therapy program.
• Current state OT license or actively pursuing licensure.
• Provide skilled OT services per physician orders.
• Assess patient needs and plan, evaluate and modify care.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer treatment programs with goals aligned to patient capacity.
• Educate patients and families on occupational therapy methods; document patient status.

A great SNF / LTC is looking for an experienced or newly graduated Occupational Therapist / OT to join their growing team for a contract assignment!

Responsibilities

• Assessment of referred patients that require rehabilitation services, providing direct patient care to assess medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.
• Establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician.
• Set realistic and achievable goals for patients, document and record the patient's condition, and educate patients and families in an appropriate occupational therapy method.

Requirements

• Must have graduated from an accredited school.
• Current state license as an Occupational Therapist / OT or proactively in the process of the application process for current state licensure as an Occupational Therapist / OT.
• Provide skilled occupational therapy services / interventions in accordance with physician orders.
• Assesses patient needs, plans for, evaluates and modifies care to meet goals of occupational therapy interventions.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.
