Centra Healthcare Solutions Inc is seeking an experienced or newly graduated Occupational Therapist / OT for a contract assignment in Hilo, Hawaii. You will be responsible for assessing patients and providing direct care to help them achieve rehabilitation goals.

Job Overview

A great SNF / LTC is looking for an experienced or newly graduated Occupational Therapist / OT to join their growing team for a contract assignment!

Responsibilities

• Assess referred patients that require rehabilitation services, providing direct patient care to evaluate medical condition, functional capabilities, limitations, restrictions, and potential for rehabilitation.
• Establish and administer a treatment program with specific goals determined according to the patient’s capacity and tolerance under the direction of the Physician.
• Set realistic and achievable goals for patients, document and record the patient’s condition, and educate patients and families in an appropriate occupational therapy method.
• Provide skilled occupational therapy services/interventions in accordance with physician orders.
• Assess patient needs, plan, evaluate, and modify care to meet goals of occupational therapy interventions.
• Collaborate with all disciplines to plan and evaluate team goals for each patient.

Qualifications

• Must have graduated from an accredited school.
• Current state license as an Occupational Therapist / OT or proactively in the process of the application process for current state licensure as an Occupational Therapist / OT.
• Current CPR certification.
