Centra Healthcare Solutions Inc. is seeking an Occupational Therapist for a contract assignment in Hawaii. The OT will deliver skilled therapy services in accordance with physician orders, assess patient needs, and collaborate with multiple disciplines to meet patient goals.

This role requires an accredited OT program graduate with an active or soon-to-be-licensed OT credential and CPR certification, serving SNF settings on a contract basis.

Qualifications

• Graduated from an accredited Occupational Therapy program.
• Current state OT license or active licensure application.
• CPR certification is required.
• Provides skilled OT services per physician orders.
• Assesses patient needs and plans, evaluates, and modifies OT interventions.
• Collaborates with all disciplines to meet patient goals.

A great Skilled Nursing Facility (SNF) is looking for an experienced or newly graduated Occupational Therapist / OT to join their growing team for a contract assignment!This is a contract position.Days schedule.This role assumes responsibility and accountability for incorporating the vision, values, mission and critical goals of the organization into their job performance.

Requirements

• Must have graduated from an accredited school.
• Current state license as an Occupational Therapist / OT or proactively in the process of the application process for current state licensure as an Occupational Therapist / OT.
• Current CPR certification.
• Provide skilled occupational therapy services / interventions in accordance with physician orders.
• Assesses patient needs, plans for, evaluates and modifies care to meet goals of occupational therapy interventions.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.
