Centra Healthcare Solutions Inc is seeking an experienced or newly graduated Occupational Therapist to join a SNF/LTC in Hazel Crest, IL on a contract assignment. This is a dayshift role with accountability for delivering high-quality OT care.

Responsibilities include providing skilled OT services per physician orders, assessing patient needs, planning and evaluating care, and collaborating with the care team to meet therapy goals.

Qualifications

• Graduated from an accredited occupational therapy program.
• Current state licensure as OT or actively pursuing licensure.

A great SNF / LTC is looking for an experienced or newly graduated Occupational Therapist / OT to join their growing team for a contract assignment! This is a contract position. Days shift. This role assumes responsibility and accountability for incorporating the vision, values, mission and critical goals of the organization into their job performance.

Requirements include:

• Must have graduated from an accredited school.


• Current state license as an Occupational Therapist / OT or proactively in the process of the application process for current state licensure as an Occupational Therapist / OT.


• Current CPR certification.

Responsibilities

• Provide skilled occupational therapy services / interventions in accordance with physician orders.


• Assesses patient needs, plans for, evaluates and modifies care to meet goals of occupational therapy interventions.


• Collaborates with all disciplines to plan and evaluate team goals for each patient.
