Job summary

Centra Healthcare Solutions is seeking an Occupational Therapist for a SNF/LTC setting on a direct hire/permanent basis. You will provide skilled OT services according to physician orders, assessing patient needs and adjusting care plans while collaborating with the multidisciplinary team.

Qualifications

• Must have graduated from an accredited school.
• Current state license as an Occupational Therapist / OT or in the licensure process.
• Current CPR certification.

• Collaborate with all disciplines to plan and evaluate team goals.

A great SNF / LTC is looking for an experienced or newly graduated Occupational Therapist / OT to join their growing team for a perm / direct hire assignment!

• This is a direct hire / 'perm' position.
• This role assumes responsibility and accountability for incorporating the vision, values, mission and critical goals of the organization into their job performance.

Requirements include:

• Must have graduated from an accredited school.
• Current state license as an Occupational Therapist / OT or proactively in the process of the application process for current state licensure as an Occupational Therapist / OT.
• Current CPR certification.
• Provide skilled occupational therapy services / interventions in accordance with physician orders.
• Assesses patient needs, plans for, evaluates and modifies care to meet goals of occupational therapy interventions.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.
