Centra Healthcare Solutions is seeking an Occupational Therapist for a Skilled Nursing Facility on a direct hire basis in the United States. The OT will provide skilled therapy services according to physician orders and work with the care team to achieve patient goals.

Ideal candidates include new graduates or experienced therapists with a current state OT license and CPR certification, focusing on patient-centered interventions and ongoing care plan improvement.

• Graduated from an accredited OT program.
• Current state licensure or active application process for OT licensure.
• Current CPR certification.

A great Skilled Nursing Facility (SNF) is looking for an experienced or newly graduated Occupational Therapist / OT to join their growing team for a direct hire / 'perm' assignment!

• This is a direct hire / 'perm' position.


• This role assumes responsibility and accountability for incorporating the vision, values, mission and critical goals of the organization into their job performance.

Requirements include:

• Must have graduated from an accredited school.


• Current state license as an Occupational Therapist / OT or proactively in the process of the application process for current state licensure as an Occupational Therapist / OT.


• Current CPR certification.


• Provide skilled occupational therapy services / interventions in accordance with physician orders.


• Assesses patient needs, plans for, evaluates and modifies care to meet goals of occupational therapy interventions.


• Collaborates with all disciplines to plan and evaluate team goals for each patient.
