Ultimate Therapy Services is hiring a full-time Occupational Therapist for Autumn Lake at Union in Union, NJ. This role focuses on evaluating residents, developing goals, and delivering therapy to improve self-care and functional abilities in a skilled nursing setting.

Clinicians collaborate with nursing and rehab teams, supervise COTA services, and contribute to discharge planning. NJ OT licensure is required; new graduates are welcome to apply.

Qualifications

• Active New Jersey Occupational Therapist license.
• Strong evaluation, treatment planning, and clinical reasoning skills.
• Clear communication and accurate, timely documentation.

Responsibilities

• Complete occupational therapy evaluations to identify residents' strengths and barriers to daily activity.
• Develop measurable goals and individualized treatment plans based on functional needs and resident priorities.
• Provide therapy addressing self-care, upper-extremity function, coordination, cognition, and activity tolerance.
• Introduce adaptive equipment and compensatory techniques that help residents perform tasks safely.
• Educate families and caregivers, supervise COTA services, and adjust treatment plans as residents progress.
• Document clinical care and contribute recommendations for discharge equipment, assistance, and continued rehabilitation.

Build treatment around what matters to each resident-from managing personal care to returning to familiar routines-while working alongside nursing and rehabilitation colleagues in a skilled nursing setting.

What You’ll Do
