Fairlane Gardens at Reading is seeking a dedicated and compassionate Occupational Therapist (OT) to join our rehabilitation team on a PRN basis. Provide occupational therapy evaluations, treatments, and interventions based on residents' individualized needs and goals.
Develop and implement individualized treatment plans focused on improving functional abilities and independence.
Address activities of daily living, mobility, coordination, strength, balance, cognition, and other areas impacting functional performance.
Accurately document evaluations, treatment sessions, resident progress, and outcomes.
Collaborate with interdisciplinary team members to support residents' rehabilitation goals.
Educate residents, families, and caregivers on therapeutic techniques and strategies to promote independence.
Ensure compliance with facility policies, professional standards, and regulatory requirements.
Master’s or Doctoral degree in Occupational Therapy from an accredited program.
Current Occupational Therapist (OT) license in the state of Pennsylvania.
Strong communication, clinical, and organizational skills.
Ability to work effectively as part of a collaborative care team.
