Job summary

Fairlane Gardens at Reading in Reading, Pennsylvania, seeks a compassionate Occupational Therapist (OT) to join our rehabilitation team on a PRN basis. You will assess residents, develop personalized treatment plans, and help improve daily functioning and independence.

In this flexible role, you’ll document progress, collaborate with an interdisciplinary team, and educate residents and families on therapeutic techniques while upholding regulatory standards and high-quality care.

Qualifications

• Master’s or Doctoral degree in Occupational Therapy.
• Active OT license in Pennsylvania.
• Strong communication, clinical, and organizational skills.
• Ability to work effectively as part of a collaborative care team.
• Commitment to compassionate, resident-centered care.

Responsibilities

• Provide occupational therapy evaluations, treatments, and interventions based on residents' individualized needs and goals.
• Develop and implement individualized treatment plans focused on improving functional abilities and independence.
• Address activities of daily living, mobility, coordination, strength, balance, cognition, and other areas impacting functional performance.
• Accurately document evaluations, treatment sessions, resident progress, and outcomes.
• Collaborate with interdisciplinary team members to support residents' rehabilitation goals.
• Educate residents, families, and caregivers on therapeutic techniques and strategies to promote independence.
• Ensure compliance with facility policies, professional standards, and regulatory requirements.
