Job summary

Lake City Healthcare and Rehabilitation in Lake City, FL is seeking a compassionate Occupational Therapist to join our rehabilitation team on a PRN basis. Responsibilities

• Perform comprehensive occupational therapy evaluations and assessments.

• Monitor resident progress and adjust treatment plans as appropriate.
• Educate residents, families, and caregivers on therapy techniques and home exercise programs.
• Maintain accurate, timely, and compliant clinical documentation.
• Collaborate with physicians, nursing staff, speech and physical therapists, and other interdisciplinary team members.
• Ensure compliance with all federal, state, facility, and professional standards.
• Foster a positive, resident-centered environment promoting dignity and independence.

Lake City Healthcare and Rehabilitation is seeking a compassionate and dedicated Occupational Therapist to join our rehabilitation team on a PRN basis. In this role, you will help residents restore independence, improve functional abilities, and achieve their highest level of performance through personalized, evidence-based therapy interventions.

Key Responsibilities

• Perform comprehensive occupational therapy evaluations and assessments.
• Develop individualized treatment plans based on each resident's goals and clinical needs.
• Provide evidence-based therapeutic interventions to improve activities of daily living (ADLs), mobility, strength, coordination, and cognitive function.
• Monitor resident progress and adjust treatment plans as appropriate.
• Educate residents, families, and caregivers on therapy techniques and home exercise programs.
• Maintain accurate, timely, and compliant clinical documentation.
• Collaborate with physicians, nursing staff, speech and physical therapists, and other interdisciplinary team members.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current and active Florida Occupational Therapist license.
• Active Level 2 Background Screening (or ability to obtain).
• CPR certification preferred.
• Previous experience in a skilled nursing, rehabilitation, or long-term care setting is preferred.
• Strong communication, organizational, and clinical decision-making skills.
