Job summary

The Chatham Hills Subacute Care Center is seeking an Occupational Therapist to evaluate patients, develop and implement rehabilitation programs, and coordinate with the care team to achieve patient-centered outcomes.

The role requires state licensure, adherence to documentation standards, and collaboration with other disciplines to support long-term and geriatric care needs.

Qualifications

• Must have graduated from an accredited Occupational Therapist program.
• Must hold a valid OT license and state registration.
• Experience supervising Occupational Therapy Assistants.
• Experience in long-term or geriatric care preferred.
• Strong documentation skills (progress notes, re-certification, discharge).

Responsibilities

• Assess referred patients needing rehabilitation and provide direct patient care to evaluate function and rehab potential.
• Establish and administer individualized treatment programs with goals aligned to patient capacity.
• Set realistic goals, document patient status, and educate patients and families in appropriate therapy methods.
• Collaborate with other disciplines to provide patient-centered care and ensure clinical excellence.
• Maintain timely documentation and reports; adhere to confidentiality and regulatory standards.

Occupational Therapist

The Chatham Hills Subacute Care Center for a talented and hard-working Occupational Therapist to join our ever-growing team.

Responsibilities Include:

The Occupational Therapist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Occupational Therapist will establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician. As well, the Occupational Therapist must set realistic and achievable goals for their patients, document and record the patient's condition and educate patients and families in an appropriate therapy method.

Qualifications/Requirements:

• Must have graduated from an accredited Occupational Therapist college or university.


• Must have a valid professional license and registration for state of practice.


• The Occupational Therapist is responsible for overseeing and supervising Occupational Therapy Assistants.


• Experience with long‑term care and/or geriatric care is preferred but not limited to.


• Well versed with documentation writing


• Evaluations, weekly progress notes, re‑certification and discharge documentations.


• Collaborates with all disciplines to provide best patient centered treatment and care.


• Exceptional patient service skills and ability to demonstrate clinical excellence


• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.


• Perform all other duties assigned by Director of Rehabilitation


• Compliance with confidentiality, HIPPA and Federal Residents Rights.

Shift Needs: Weekdays and Weekends
