Job summary

Apploi is seeking a dedicated Occupational Therapist to join the team at Chatham Hills Subacute Care Center. You will assess referred patients, develop and implement rehabilitation plans, and set realistic goals based on each patient’s capacity under physician direction.

You will document patient progress, educate patients and families, supervise Occupational Therapy Assistants, and collaborate with interdisciplinary staff to deliver high‑quality, patient‑centered care in a long‑term care

Qualifications

• Must graduate from an accredited OT program.
• Must hold a valid state license and registration.
• Experience with long‑term care and/or geriatric care preferred.
• Strong documentation skills and progress notes.
• Collaborates with disciplines to provide patient‑centered care.
• Excellent patient service and professionalism.
• Supervise Occupational Therapy Assistants.
• HIPAA and confidentiality compliance.

Responsibilities

• Assess referred patients requiring rehabilitation services.
• Provide direct patient care and evaluate medical condition and functional capabilities.
• Establish and administer treatment programs with goals per patient capacity and physician direction.
• Document patient condition and progress; discharge and re‑certification as needed.
• Educate patients and families in therapy methods.
• Collaborate with all disciplines to ensure best patient‑centered care.
• Maintain professional standards and attend to scheduling and confidentiality.

Occupational Therapist

The Chatham Hills Subacute Care Center for a talented and hard-working Occupational Therapist to join our ever-growing team.

Responsibilities Include:

The Occupational Therapist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Occupational Therapist will establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician. As well, the Occupational Therapist must set realistic and achievable goals for their patients, document and record the patient's condition and educate patients and families in an appropriate therapy method.

Qualifications/Requirements:

• Must have graduated from an accredited Occupational Therapist college or university.


• Must have a valid professional license and registration for state of practice.


• The Occupational Therapist is responsible for overseeing and supervising Occupational Therapy Assistants.


• Experience with long‑term care and/or geriatric care is preferred but not limited to.


• Well versed with documentation writing


• Evaluations, weekly progress notes, re‑certification and discharge documentations.


• Collaborates with all disciplines to provide best patient centered treatment and care.


• Exceptional patient service skills and ability to demonstrate clinical excellence


• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.


• Perform all other duties assigned by Director of Rehabilitation


• Compliance with confidentiality, HIPPA and Federal Residents Rights.
