Job summary

The Chatham Hills Subacute Care Center in New Jersey is seeking a dedicated Occupational Therapist to join our growing rehabilitation team. You will assess patients, establish and monitor therapy plans, and educate families on treatment approaches while supervising aides and collaborating with the care team.

Responsibilities include patient assessments, setting goals, documenting progress, and ensuring high-quality patient-centered care in a multidisciplinary environment.

Qualifications

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• Oversee and supervise Occupational Therapy Assistants.
• Experience with long‑term care and/or geriatric care preferred.
• Well-versed in documentation writing.
• Routinely perform evaluations, weekly progress notes, re-certification and discharge documentation.
• Collaborate with all disciplines to provide patient-centered care.
• Demonstrate exceptional patient service and clinical excellence.
• Punctuality and professionalism in schedule adherence.
• Perform duties as directed by the Director of Rehabilitation.
• Comply with confidentiality, HIPAA and Federal Residents Rights.

Occupational Therapist

The Chatham Hills Subacute Care Center for a talented and hard-working Occupational Therapist to join our ever-growing team.

Responsibilities Include:

The Occupational Therapist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Occupational Therapist will establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician. As well, the Occupational Therapist must set realistic and achievable goals for their patients, document and record the patient's condition and educate patients and families in an appropriate therapy method.

Qualifications/Requirements:

• Must have graduated from an accredited Occupational Therapist college or university.

• The Occupational Therapist is responsible for overseeing and supervising Occupational Therapy Assistants.


• Experience with long‑term care and/or geriatric care is preferred but not limited to.


• Well versed with documentation writing

• Collaborates with all disciplines to provide best patient centered treatment and care.


• Exceptional patient service skills and ability to demonstrate clinical excellence


• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.


• Perform all other duties assigned by Director of Rehabilitation


• Compliance with confidentiality, HIPPA and Federal Residents Rights.
