Apploi's Chatham Hills Subacute Care Center is seeking a skilled Occupational Therapist to join our rehabilitative team. You will assess referred patients, develop and implement individualized treatment plans, and work under the direction of the Physician to help patients regain function.

You will supervise Occupational Therapy Assistants, document progress, educate patients and families, and collaborate with the interdisciplinary team to deliver patient-centered care.

Formación

• Must have graduated from an accredited Occupational Therapist program.
• Must hold a valid professional license and registration for the state of practice.
• Overseeing and supervising Occupational Therapy Assistants.
• Experience with long-term care and geriatric care is preferred.
• Strong documentation writing skills.

Responsabilidades

• Assess referred patients needing rehabilitation services and determine functional status.
• Establish and administer a treatment program with specific goals.
• Set realistic, achievable goals and monitor patient tolerance per physician guidance.
• Document patient condition and progress, including discharge planning.
• Educate patients and families in appropriate therapy methods.
• Collaborate with all disciplines to deliver patient-centered care.
• Perform duties as assigned by the Director of Rehabilitation.

Occupational Therapist

The Chatham Hills Subacute Care Center for a talented and hard-working Occupational Therapist to join our ever-growing team.

Responsibilities Include:

The Occupational Therapist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Occupational Therapist will establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician. As well, the Occupational Therapist must set realistic and achievable goals for their patients, document and record the patient's condition and educate patients and families in an appropriate therapy method.

Qualifications/Requirements:

• Must have graduated from an accredited Occupational Therapist college or university.


• Must have a valid professional license and registration for state of practice.


• The Occupational Therapist is responsible for overseeing and supervising Occupational Therapy Assistants.


• Experience with long‑term care and/or geriatric care is preferred but not limited to.


• Well versed with documentation writing


• Evaluations, weekly progress notes, re‑certification and discharge documentations.


• Collaborates with all disciplines to provide best patient centered treatment and care.


• Exceptional patient service skills and ability to demonstrate clinical excellence


• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.


• Perform all other duties assigned by Director of Rehabilitation


• Compliance with confidentiality, HIPPA and Federal Residents Rights.

Shift Needs: Weekdays and Weekends
