Job summary

The Chatham Hills Subacute Care Center is seeking a qualified Occupational Therapist to join our rehabilitation team in New Jersey. You will assess patients, develop and supervise treatment plans, and coordinate with physicians and staff to optimize recovery.

Requirements include graduation from an accredited OT program, valid NJ license, and experience in long-term/geriatric care. You will document progress, educate patients and families, and supervise OT Assistants as needed.

Qualifications

• Graduated from an accredited Occupational Therapist college or university.
• Licensed and registered to practice in state of New Jersey.
• Experience supervising Occupational Therapy Assistants.
• Experience in long‑term care and/or geriatric care preferred.

Responsibilities

• Assess referred patients for rehabilitation needs and provide direct patient care.
• Establish and manage treatment programs with patient-specific goals.
• Document patient condition, progress notes, and discharge planning.
• Educate patients and families on therapy methods.
• Collaborate with multidisciplinary team to provide patient-centered care.
• Report to work on time and maintain professional standards.

Occupational Therapist

The Chatham Hills Subacute Care Center for a talented and hard-working Occupational Therapist to join our ever-growing team.

Responsibilities Include:

The Occupational Therapist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Occupational Therapist will establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician. As well, the Occupational Therapist must set realistic and achievable goals for their patients, document and record the patient's condition and educate patients and families in an appropriate therapy method.

Qualifications/Requirements:

• Must have graduated from an accredited Occupational Therapist college or university.


• Must have a valid professional license and registration for state of practice.


• The Occupational Therapist is responsible for overseeing and supervising Occupational Therapy Assistants.


• Experience with long‑term care and/or geriatric care is preferred but not limited to.


• Well versed with documentation writing


• Evaluations, weekly progress notes, re‑certification and discharge documentations.


• Collaborates with all disciplines to provide best patient centered treatment and care.


• Exceptional patient service skills and ability to demonstrate clinical excellence


• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.


• Perform all other duties assigned by Director of Rehabilitation


• Compliance with confidentiality, HIPPA and Federal Residents Rights.

Shift Needs: Weekdays and Weekends
