Job summary

Chatham Hills Subacute Care Center is seeking a dedicated Occupational Therapist to join our rehabilitation team in New Jersey. The OT will assess referred residents, develop and supervise treatment plans, and work under physician direction to promote functional recovery.

Responsibilities include goal setting, progress documentation, patient and family education, supervising OTAs, and collaborating with a multidisciplinary team while maintaining confidentiality and professional standards.

Qualifications

• Graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• Overseeing and supervising Occupational Therapy Assistants.
• Experience with long‑term care and/or geriatric care is preferred but not required.
• Strong documentation writing skills.
• Evaluations, weekly progress notes, re‑certification and discharge documentation.
• Collaborates with all disciplines to provide patient-centered care.
• Professionalism and timely attendance are expected.

Responsibilities

• Assess referred patients to determine rehabilitation needs and potential.
• Establish and administer treatment plans with goals aligned to patient capacity.
• Set realistic and achievable goals and document the patient’s condition.
• Educate patients and families in appropriate therapy methods.
• Supervise Occupational Therapy Assistants and collaborate with the rehab team.
• Maintain confidentiality and comply with HIPAA and residents’ rights.

Occupational Therapist

The Chatham Hills Subacute Care Center for a talented and hard-working Occupational Therapist to join our ever-growing team.

Responsibilities Include:

The Occupational Therapist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Occupational Therapist will establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician. As well, the Occupational Therapist must set realistic and achievable goals for their patients, document and record the patient's condition and educate patients and families in an appropriate therapy method.

Qualifications/Requirements:

• Must have graduated from an accredited Occupational Therapist college or university.

• The Occupational Therapist is responsible for overseeing and supervising Occupational Therapy Assistants.

• Well versed with documentation writing

• Collaborates with all disciplines to provide best patient centered treatment and care.


• Exceptional patient service skills and ability to demonstrate clinical excellence


• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.


• Perform all other duties assigned by Director of Rehabilitation


• Compliance with confidentiality, HIPPA and Federal Residents Rights.
