Job summary

The Chatham Hills Subacute Care Center is seeking a talented Occupational Therapist to join our rehabilitation team. You will assess patients, develop and administer treatment programs, set goals, document progress, and educate patients and families under physician direction.

Responsibilities include supervising OT Assistants, coordinating with multidisciplinary teams, and ensuring high-quality care with timely documentation and discharge planning. Weekdays and weekends shift needs are noted.

Qualifications

• Graduated from an accredited OT college or university.
• Valid professional license and registration for state of practice.
• Experience supervising Occupational Therapy Assistants.
• Experience in long-term care and/or geriatric care preferred.
• Well-versed in documentation writing; ability to complete evaluations, weekly progress notes, re-certifications and discharge documents.

Responsibilities

• Assess referred patients to determine rehabilitation needs and potential for rehabilitation.
• Establish and administer treatment programs with goals tailored to patient capacity and tolerance.
• Set realistic and achievable goals; document patient condition and educate patients and families.
• Collaborate with all disciplines to provide patient-centered care.
• Perform all other duties as directed by the Director of Rehabilitation.

Occupational Therapist

The Chatham Hills Subacute Care Center for a talented and hard-working Occupational Therapist to join our ever-growing team.

Responsibilities Include:

The Occupational Therapist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Occupational Therapist will establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician. As well, the Occupational Therapist must set realistic and achievable goals for their patients, document and record the patient's condition and educate patients and families in an appropriate therapy method.

Qualifications/Requirements:

• Must have graduated from an accredited Occupational Therapist college or university.


• Must have a valid professional license and registration for state of practice.


• The Occupational Therapist is responsible for overseeing and supervising Occupational Therapy Assistants.


• Experience with long‑term care and/or geriatric care is preferred but not limited to.


• Well versed with documentation writing


• Evaluations, weekly progress notes, re‑certification and discharge documentations.


• Collaborates with all disciplines to provide best patient centered treatment and care.


• Exceptional patient service skills and ability to demonstrate clinical excellence


• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.


• Perform all other duties assigned by Director of Rehabilitation


• Compliance with confidentiality, HIPPA and Federal Residents Rights.
