Job summary

The Chatham Hills Subacute Care Center is seeking a dedicated Occupational Therapist to join our rehabilitation team. You will assess patients, plan and implement treatment programs, and collaborate with physicians and staff to optimize function for long-term care residents.

You will supervise OT Assistants, document progression, and educate patients and families on therapy approaches while maintaining HIPAA compliance and delivering compassionate, patient-centered care.

Qualifications

• Must have graduated from an accredited Occupational Therapist program.
• Must possess a valid professional OT license and registration for the state of practice.
• Experience with long-term care and/or geriatric care is preferred.
• Strong documentation writing skills are required.
• Evaluations, progress notes, re-certification and discharge documentation are expected.

Responsibilities

• Assess referred patients requiring rehabilitation services and provide direct patient care.
• Establish and administer treatment programs with goals based on patient capacity and tolerance.
• Document patient condition and educate patients and families in therapy methods.
• Supervise and oversee Occupational Therapy Assistants.
• Collaborate with all disciplines to provide patient-centered care.

Occupational Therapist

The Chatham Hills Subacute Care Center for a talented and hard-working Occupational Therapist to join our ever-growing team.

Responsibilities Include:

The Occupational Therapist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Occupational Therapist will establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician. As well, the Occupational Therapist must set realistic and achievable goals for their patients, document and record the patient's condition and educate patients and families in an appropriate therapy method.

Qualifications/Requirements:

• Must have graduated from an accredited Occupational Therapist college or university.


• Must have a valid professional license and registration for state of practice.


• The Occupational Therapist is responsible for overseeing and supervising Occupational Therapy Assistants.


• Experience with long‑term care and/or geriatric care is preferred but not limited to.


• Well versed with documentation writing


• Evaluations, weekly progress notes, re‑certification and discharge documentations.


• Collaborates with all disciplines to provide best patient centered treatment and care.


• Exceptional patient service skills and ability to demonstrate clinical excellence


• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.


• Perform all other duties assigned by Director of Rehabilitation


• Compliance with confidentiality, HIPPA and Federal Residents Rights.

Shift Needs: Weekdays and Weekends
