Job summary

Chatham Hills Subacute Care Center in Livingston, NJ is seeking a dedicated Occupational Therapist to join our growing rehab team. The OT will assess patients referred for rehabilitation, design treatment plans and deliver direct care under physician direction.

The role requires establishing realistic goals, documenting patient progress, educating patients and families, and supervising OT Assistants. Collaboration with all disciplines and HIPAA compliance are essential.

Qualifications

• Graduated from an accredited Occupational Therapy program.
• Valid OT license and state registration.
• Experience with long-term/geriatric care preferred.
• Strong documentation and record-keeping.
• Ability to complete evaluations and progress notes.

Responsibilities

• Assess referred patients requiring rehabilitation.
• Provide direct patient care and set treatment goals.
• Develop and administer patient-specific rehab plans.
• Educate patients and families on therapy approaches.
• Oversight of Occupational Therapy Assistants.
• Coordinate with other disciplines to ensure patient-centered care.
• Maintain confidential records and HIPAA compliance.

Occupational Therapist

The Chatham Hills Subacute Care Center for a talented and hard-working Occupational Therapist to join our ever-growing team.

Responsibilities Include:

The Occupational Therapist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Occupational Therapist will establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician. As well, the Occupational Therapist must set realistic and achievable goals for their patients, document and record the patient's condition and educate patients and families in an appropriate therapy method.

Qualifications/Requirements:

• Must have graduated from an accredited Occupational Therapist college or university.


• Must have a valid professional license and registration for state of practice.


• The Occupational Therapist is responsible for overseeing and supervising Occupational Therapy Assistants.


• Experience with long‑term care and/or geriatric care is preferred but not limited to.


• Well versed with documentation writing


• Evaluations, weekly progress notes, re‑certification and discharge documentations.


• Collaborates with all disciplines to provide best patient centered treatment and care.


• Exceptional patient service skills and ability to demonstrate clinical excellence


• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.


• Perform all other duties assigned by Director of Rehabilitation


• Compliance with confidentiality, HIPPA and Federal Residents Rights.

Shift Needs: Weekdays and Weekends
