Job summary

Chatham Hills Subacute Care Center is seeking an Occupational Therapist to join our rehabilitation team. The role involves examining referred patients, delivering direct therapy, and building customized treatment plans with achievable goals appropriate to each patient’s abilities.

The OT will document progress, educate families, and collaborate with physicians and other care team members to ensure high-quality patient-centered care. Prior experience in long-term or geriatric care is preferred.

Qualifications

• Graduated from an accredited Occupational Therapy program.
• Must hold valid professional license and state registration.
• Experience with long-term care or geriatric settings preferred.
• Strong documentation writing skills and recordkeeping.

Responsibilities

• Assess referred patients to determine rehabilitation needs and eligibility.
• Provide direct patient care and implement treatment plans.
• Develop and monitor individualized goals based on patient capacity and tolerance.
• Document patient condition, progress, and discharge plans; educate patients and families.
• Coordinate with physicians and other disciplines to optimize care.

Occupational Therapist

The Chatham Hills Subacute Care Center for a talented and hard-working Occupational Therapist to join our ever-growing team.

Responsibilities Include:

The Occupational Therapist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Also, the Occupational Therapist will establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician. As well, the Occupational Therapist must set realistic and achievable goals for their patients, document and record the patient's condition and educate patients and families in an appropriate therapy method.

Qualifications/Requirements:

• Must have graduated from an accredited Occupational Therapist college or university.


• Must have a valid professional license and registration for state of practice.


• The Occupational Therapist is responsible for overseeing and supervising Occupational Therapy Assistants.


• Experience with long‑term care and/or geriatric care is preferred but not limited to.


• Well versed with documentation writing


• Evaluations, weekly progress notes, re‑certification and discharge documentations.


• Collaborates with all disciplines to provide best patient centered treatment and care.


• Exceptional patient service skills and ability to demonstrate clinical excellence


• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.


• Perform all other duties assigned by Director of Rehabilitation


• Compliance with confidentiality, HIPPA and Federal Residents Rights.

Shift Needs: Weekdays and Weekends
