Job summary

One Home Health in Indiana is seeking a part-time Licensed Occupational Therapist to provide in-home evaluations and therapy across Lake and Porter counties and surrounding areas. You will develop individualized care plans, educate patients and caregivers, and document visits accurately.

Join a team with flexible scheduling, patient-centered care focus, and supportive clinical leadership. Prior home health experience is preferred, with licensure in good standing.

Qualifications

• Active licensure as an Occupational Therapist (OT) in good standing
• Prior home health experience preferred; new candidates with strong clinical skills encouraged to apply

• Demonstrated ability to work independently while effectively managing a flexible schedule

Responsibilities

• Conduct comprehensive in-home occupational therapy evaluations and deliver targeted treatment interventions
• Develop and implement individualized care plans designed to maximize patient independence and functional outcomes
• Educate patients and caregivers on safety practices, adaptive techniques, and proper use of assistive equipment
• Complete accurate, timely, and compliant clinical documentation for all patient visits
• Collaborate effectively with the interdisciplinary team to communicate patient progress and adjust care plans as needed
• Ensure adherence to all clinical, regulatory, and documentation standards

About the Role

We are seeking a part-time licensed Occupational Therapist (OT) to join our exceptional team in Indiana. We have openings in Lake County (Crown Point, Hobart, Gary, Merrillville, Munster, etc.), Porter County (Valparaiso, Portage, Porter, Boone Grove, South Haven, etc.), and the surrounding areas.
